Evaluation for Essentials A

1. Please be sure each person fills out the evaluation form and returns it to you.

2. If attendees want ceu's for the class, please send me a sign up sheet with each person's name typed or printed LEGIBLY, with a signature next to it, and the last 4 digits of each person's social security number. If we can't read it or don't have all the information, we will not issue the certificate.

3. Please STRONGLY ENCOURAGE everyone to join the meldatabases listserv at this URL
