Reimbursement Claims for Lost MeLCat Materials
Due to severe budget reductions, after July 31, 2010 the Library of Michigan no longer provides reimbursements to libraries for lost MeLCat materials.
The Library of Michigan accepted reimbursement claims for lost MeLCat loan material through July 31, 2010. Claims were accepted during the month of July only and had to be postmarked by July 31, 2010.
Payments for lost MeLCat materials were made once annually in July. Note: If there are insufficient funds to cover the cost of all lost materials at 100% of the claimed amount, the Library of Michigan will prorate payments at an equitable percentage of the claimed amount.
A MeLCat Borrowing Library was able to file for reimbursement in cases where all of the following conditions exist:
- All means of obtaining reimbursement from the patron have been exhausted and there is no reason to believe the billed patron will ever return the item or pay for it.
- The Supplying Library has invoiced the Borrowing Library for the lost material.
- The Borrowing Library has paid the Supplying Library for the lost material.
Borrowing Library Procedure for Filing a Reimbursement Claim for Lost MeLCat Materials
- Print out a "MeLCat Materials Reimbursement Claim Form."
- Complete the Claim Form providing your library name, address and phone number, the date of claim (July dd, yyyy), and the name and signature of the person filing the claim form. For each lost item provide the following:
- Name of the Supplying Library
- Title, author, publication date, and format of the lost material
- Amount paid to the Supplying Library for that item
- Amount of processing fees or sales tax paid
- Amount of replacement cost of item
- Attach a copy of the paid invoice for each item listed on the claim form.
- Mail the claim form and attachment(s) to:
